$1,000.00 INSURANCE COVERAGE PAID FOR YOU BY KEMBA CREDIT UNION

As a Kemba Credit Union member age 18 or older you can help protect your family’s financial future with Accidental Death and Dismemberment (AD&D) Insurance underwritten by Minnesota Life Insurance Company. In addition to the $1,000.00 Credit Union Paid coverage, you can purchase Additional Coverage at group rates.

  • You are pre-authorized for Additional Coverage up to $300,000
  • Your acceptance is guaranteed
  • Benefits are paid to your beneficiary if you die from a covered accident

You cannot be turned down for the Credit Union Paid Coverage — or any additional coverage you may select. All coverage reduces by 50% at age 70 and older, regardless of age at enrollment.

Remember, the $1,000.00 coverage is at no cost to you. It’s complimentary of Kemba Credit Union.

Are there other ways to enroll?

Simply call 1.844.269.1292 toll free and a friendly representative will assist you. Once you receive your Certificate of Insurance in the mail, you can request additional coverage of up to $300,000.00 for you. You can request coverage for your family as well.

How much additional coverage may I choose?

You alone can decide how much coverage you need. This AD&D Insurance has four coverage levels from $50,000.00 to $300,000.00 all available at group rates. You may want to consult a trusted advisor as to how much coverage is right for you.

Can I get insurance for my spouse and family, too? 

Spouses and unmarried dependent children may be insured under the family plan. If you choose the family plan, spouses are insured for 50% of your additional coverage amount. Unmarried dependent children are each insured for 20%, regardless of the number of dependent children. If there are no dependent children, their spouse will be insured for 60% of the additional coverage. 

Do I have to take a medical exam or answer a lot of health questions?

No medical exams, blood tests, or health questions are required to get and keep this coverage.

Minnesota Life Insurance Company is the underwriter.

Are there any additional benefits?

You automatically receive the following benefits when you select added coverage:

  • Common Carrier - this benefit pays twice the elected additional coverage if your loss occurs due to a covered accident while you are in, entering, or exiting a common carrier.

  • Education Benefit - if you suffer an accidental death, this benefit reimburses education expenses of 2% of the insured's accidental death benefit for each dependent child who is enrolled as a full-time student in an institution of higher learning.

  • Anti-Inflation Benefit - for every two years an insured's additional coverage remains continuously in force, it will be increased by 5% until it reaches 125% of the original amount.

  • Adaptive Home and Vehicle Benefit - provides a benefit if the insured's home and/or private motor vehicle requires modifications to accommodate the insured's dismemberment or total and permanent disability resulting from a covered loss.

  • Waiver of Premium Benefit - if an insured becomes totally and permanently disabled prior to age 60, remains continuously disabled for at least 6 months, and continues to pay premiums during this 6 month waiting period, the insured's accidental death and dismemberment insurance will remain in force after the waiting period without payment of premium.

What is a “Common Carrier”?

Common Carrier includes any public air, land, or water conveyance operated under a license for commercial passenger service.

What is a private motor vehicle?

Private motor vehicle means a four-wheeled, private passenger car, station wagon, pick-up truck, van, sport utility vehicle or jeep-type automobile, which is not being used as a common carrier.

What coverage is provided by this insurance?

This insurance provides Accidental Death and Dismemberment benefits in the event of a death or loss of life, limb, sight, speech, or hearing as the result of a covered accident.

Who is covered under the family plan?

The Family Plan would provide coverage for yourself, your spouse and any eligible dependent children that you may have. Please refer to your Insurance Coverage Document for more detailed information.

Is there an age cutoff?

There is no termination age; however, all coverage reduces by 50% at age 70 and older, regardless of age at enrollment.

Does this policy have a cash value?

No. There is no cash value for this insurance. 

What if I have other insurance?

This insurance will pay all benefits due to a covered accident regardless of any other insurance that you may have.

Who can I name as my beneficiary?

Name anyone or as many beneficiaries as you would like.

Are on-the-job accidents covered?

Yes. Your coverage is effective 24 hours a day, anywhere in the world.

Who is Franklin Madison Group LLC? Who is MyOnlineInsurance? 

Franklin Madison Group LLC is the Plan Administrator for this Kemba Credit Union-sponsored insurance coverage. Kemba Credit Union is our client. www.MyOnlineInsurance.com is the members-access website service, sponsored and wholly owned by Franklin Madison Group LLC.

Billing 

How will I be billed for my coverage?

There is no charge for the complimentary coverage. Kemba Credit Union pays it in full for you. Additional coverage you select will be conveniently billed to the account provided at the time of enrollment.

What day does the premium come out of my account?

About the 5th day of your billing cycle due date.

Claims

How I do I file a claim?

You can receive and complete our Insurance Claim Forms by calling our Claims Department at 1-800-252-2148. A representative would be glad to assist you in filing a claim.

How long does it take to process my claim?

For all benefits payable under the policy, the insurer will pay the Insured Person or beneficiary the applicable Benefit Amount within 60 days of receiving complete Proof of Loss.

Why do you need my hospital records to process a claim?

These records are required by the insurance company to verify that the primary cause of death/dismemberment was due to a covered accident because this is Accidental Death and Dismemberment insurance.

What if I can’t get an accident report?

Enclose name or business card of the investigating officer with your claims forms. The insurance company can then get in contact with the police directly for additional information that may be required in adjudicating your claim.
